Application Card (Form A)

1. There is a different Application Card for each application method. Please print out in color the Application Card for your application method.
2. Please use the Reference Number file to determine the reference number for your application method.

Application Documents

<Notes>

1. Please print out the Application Documents single-sided.
   ※Please fill in the reference number of the application method you use on the Application Card.

Application Envelope (front and back)

<Notes>

1. Please print out both the front and back sides.
2. On the front sheet, please fill in all necessary information (type of screening pattern, name, address etc.)
3. Please use the back sheet as a checklist for all the application documents.
4. Please securely affix the front side sheet to an A4 size envelope. Please put the back sheet (checklist) inside the envelope with all your application documents.
